Social and physical isolation is one of many challenges facing elders and disabled adults. Our center, staffed by friendly and attentive professionals, provides a comprehensive day program that engages vulnerable elders and disabled adults within a caring community. While our focus is to improve our clients’ overall quality of life, we also aim to offer respite to the caregivers we view as our community partners.

  • Facility:  Our name speaks for itself. The disabled adults day care center program begins the minute you arrive at Active Life. Our facility is designed to promote a sense of well-being through architectural details such as:
      • Comfort: Carpeting in our professional rooms, quiet rooms and all the offices

      • Warmth: Our large fireplace creates cozy, home like environment
      • Happiness: Professionally-designed wall colors and meditation garden
  • Recreational Activities: Our Activity Director provides creative activities designed to stimulate your mind and muscles leaving you with joy and fulfillment each day.
  • NutritionUnder the direction of our registered dietician, we offer a full breakfast, hot lunch and afternoon snack professionally prepared for you by a local restaurant. We also provide dietary monitoring/counseling and nutrition education.
  • NursingOur inter-disciplinary team will prepare a care plan that is right for you. Under the direction of our Director of Nursing, our licensed-care team will provide evaluations, treatments, health education, medication management/administration and health monitoring. Our certified nursing aides will provide assistance when you need it with your activities of daily living (eating, toileting, bathing, dressing and locomotion).
  • Social Services: A Certified social worker will help you navigate your way through the many intricacies of obtaining the assistance you are entitled to. They will also schedule and coordinate transport to doctor appointments.
  • Medical Services: Our nurses help to facilitate off-site healthcare services including physician care, physical, occupational and speech therapy, podiatry, dental and vision care, psychiatry and psychology counseling.
  • Transportation: Our fleet of executive vans and buses will pick you up and drop you off every day. Our fleet can accommodate all assistive devices you may have. We also provide transportation to medical appointments, retail stores and other community services.
