Can you provide transportation?

Transportation is included in the cost of the program. Our fleet of executive vans and buses will pick you up and drop you off every day. Our fleet can accommodate all assistive devices you may have. We also provide transportation to medical appointments, retail stores and other community services. Family members can transport their loved ones if they choose.

How do I get started in the program?

Just contact you physician and our Social Service or Intake Coordinator office at (978) 400-7604 or just stop by, no appointment is necessary. Our staff will assist you.

Is this program for me?

Adult day health offers many services to help a wide variety of people.  If you need help with any of the following, you may be looking for adult day health.

• monitoring of medications, blood sugars, blood pressure or weight

• administration of medications

• socialization with others

• meals

• scheduling of appointments

• physical therapy

• occupational therapy

• use of walkers, wheelchairs, and splints

• assistance in the bathroom

• transportation to and from the program and medical appointments

What are your program hours?

We are open from 7:00 a.m. to 4:00 p.m. Monday -Saturday. Our flexible hours program allows clients to set up a schedule that will meet their needs.

What if my loved one is unsure about attending?

Active Life has trained staff to handle this situation every day. Encourage your loved one to attend our facility for a few days for free with no obligation or commitment. We are confident that once your loved one attends one day of our program, he/she will want to come back.

What is the cost?

Medicaid covers the total cost of our program. In addition, Senior Care Option (SCO) plans such as Senior Whole Health, United, Tufts, NaviCare etc also cover ADHC.

At the initial interview, funding and the cost of the program are discussed.
The following funding options are available to eligible adults:

  • Mass Health Standard
  • Senior Whole Health
  • Evercare
  • Veterans Benefits
  • Private Pay

There is no out of pocket expense for participants with any of the above listed insurance coverage.
